# Medical records from any source

> Turn a consultation, a photo of a report or a PDF into one structured, coded medical record.

This recipe turns whatever arrives at a clinic, a spoken consultation, a photo of a lab report or a PDF, into one structured medical record. It uses [Scribe](https://developer.eka.care/docs/scribe/overview), [Document Parsing](https://developer.eka.care/docs/models/medical-document-parsing.md) and [Health Records](https://developer.eka.care/docs/health-records/upload-records.md).

**1. Turn the consultation into a note**

Record or stream the consultation with [Scribe](https://developer.eka.care/docs/scribe/quickstart.md). It returns a structured note in the [template](https://developer.eka.care/api-sdk/scribe/templates/list-templates.md) you choose, and a [FHIR bundle](https://developer.eka.care/docs/scribe/resources/fhir-structure.md).

**2. Read reports, PDFs and photos**

Upload a lab report as a PDF or an image with [Document Parsing](https://developer.eka.care/docs/models/medical-document-parsing.md). It returns the values in the report as structured data.

**3. Code what was found**

Map the diagnoses, medicines and tests to standard codes with [medical entity codification](https://developer.eka.care/docs/models/medical-entity-codification.md), so other systems can read them.

**4. Keep it in the patient's record**

Save the note and the parsed report against the patient with [Health Records](https://developer.eka.care/docs/health-records/upload-records.md).

To have a coding agent build this with you, see Build with AI for [Scribe](https://developer.eka.care/docs/scribe/build-with-ai) and [Document Parsing](https://developer.eka.care/docs/models/build-with-ai.md).
