The **first item is always the rate plan line**: its `rate` is the sum of the rate plan's inclusions (per night), `quantity` is the admission duration (or `1`), and `name` is `/night`. When the admission has no rate plan, this line is a placeholder with `rate` and `quantity` of `0` and `name` of `-`. The remaining items are the admission's estimate lines. Estimate line ids are not included in this response.
